One of our Wellbeing and Social Prescribing practitioners will work with you in your GP surgery or telephonically to assess your needs. They will then make relevant referrals to any specialist organisations and services within the community. You will be supported to focus on identifying and achieving your goals and access local community resources.
This is currently available to anyone whose GP is in Barry and Sully.
This service is available to anyone over and including the age of 16.
